Our Blue Health Services programs are designed to improve the quality and outcomes of healthcare services, increase the effectiveness of healthcare delivery and control healthcare costs.

Our Blue Health Services programs provide effective patient management of chronic conditions and adherence to guidelines.  We promote the stabilization of a condition followed by:

  • Disease Education
  • Coaching
  • Behavior Modification
  • Reminders
  • Disease Monitoring

The goal is to increase the member's compliance with their physician's plan of care and improved self-care. We also put emphasis on the evaluation of the member's adherence to clinical practice guidelines.

We offer disease management programs for eligible Blue and HMOLA members as well as eligible Federal Employee Program (FEP) members. These programs support the physician's care management strategies while providing early intervention and patient compliance services to help strengthen physician-patient relationships.
